Supplements personalized for Stress, Sleep, Diet Control and Mental Focus HOME MONITORING INJECTION PLANS OR FAST TRACK PROGRAM FOR HORMONE REPLACEMENT THERAPY Home monitoring injection plans give you the opportunity to home inject with the close supervision of a hormone specialist. Clients are eligible for home injections if they meet the criteria for a stable patient. This includes transfer of care from other hormone clinics or doctors’ offices that have been stable on treatment for more than 3 months, or if the patient is new to hormone replacement therapy, stable as a Vitality client for a minimum for 3 months. Patients are required to meet with the provider a minimum of one time per month. Telemedicine visits are allowed every other month. Fast track program is offered to stable clients that do not want to self inject. This program requires one monthly visit with the provider. Patients will then have the ability to come in weekly for their injection with the Medical Assistant. This program is flexible and allows patients to come in anytime during their chosen day of the week for a quick 10 minute or less injection visit.
